THE LIFE CARE PLAN

GOAL SETTING
We work with you to determine what your family is trying
to achieve and what the needs are of your loved ones.
PLANNING PROCESS
Legal, Financial and Personal
First, we
establish the legal needs of your loved one. This may include
estate planning and preparing necessary documents such as Living Wills
and Durable Powers of Attorney.
Next,
we establish a personal plan for your loved one. We address
issues such as housing, family, health, social issues and we collect and
preserve personal information for future use.
We also make
appropriate referrals to professionals such as accountants, geriatric
care managers, financial planners etc. as needed. Based on our 20+ years
of experience, we can help you make sound choices of allied
professionals your Life Care Plan will require.
Finally, we address the financial concerns of the
family and loved one. How are we going to pay for long term care? Is
there insurance or private funds? Are public benefits such as
Medicare, Medicaid, or Veteran's benefits available?

About

Our staff of 4 trained
professional assistants including a Family Services Coordinator, who
with the attorney, assists the family in accessing resources and
advocacy.
Recognizing the value
and love of your mom, dad, or other family member, we have hired a
Family Service Counselor who will work with you to make sure that you
are getting all the services to which you are entitled. This person will
be "your" advocate whenever that need arises.
Our fees are based on a
flat fee so that you are never billed for phone calls or questions. We
outline exactly what our fees are for.

Alice Reiter Feld
is an attorney in the private practice of law in the Cities of Tamarac
and Delray Beach, Florida. She is licensed to practice in the States of
Florida and New York. Ms. Feld is Board Certified by the Florida Bar and
the National Elder Law Foundation as an elder law specialist and is "AV"
rated by Martindale Hubbell. She is the immediate past Chair of the
Elder Law section of the Broward County Bar Association. She is also the
President of the Academy of Florida Elder law Attorneys. Ms. Reiter Feld
is a 1980 graduate of St. John's University School of Law in New York.
